Home Health Care
I worked with a small home health care agency to address two distinct caregiver performance challenges.
-
Delivery of required reportable medical event training to a geographically distributed workforce without access to a learning management system.
-
Improvement of Electronic Visit Verification (EVV) timekeeping practices to reduce documentation errors and strengthen compliance.
Through analysis of the business need, audience and operating environment, two targeted solutions were developed: a mobile-friendly eLearning experience and a point-of-work performance support tool.
These solutions worked well with the existing constraints, which included a distributed workforce, limited access to technology and the lack of a Learning Management System (LMS).

For this project, I drafted a single Project Brief, describing both performance solutions. I also developed separate Design Briefs for each solution. This documentation ensures that Project Scope as well as individual Project deliverables are clear and understood by all. I have provided examples of these documents below.

Project #1 - Electronic Visit Verification (EVV) Timekeeping
Problem to be Solved?
This project was to reduce errors and strengthen compliance for TennCare CHOICES clients using the EVV timekeeping application.

What will Success look like?
The Performance Goal was to ensure that Agency Caregivers correctly clock in and out of the TennCare CHOICES system at the start and end of each visit to a CHOICES client, as well as complete the necessary documentation in real time or immediately after.

Who is the Audience?
The audience for this project are Agency non-medical caregivers with a high school (or equivalent) education, who work 20-40 hours per week in client homes with no dedicated field supervision.

What is the Solution?
In this case, a Performance Support solution was ideal. Using Canva, I created a Lanyard or Wallet Card with EVV Timekeeping instructions which will be provided to caregivers with their Agency identification badge.

Project #2 - Reportable Medical Events Training
Problem to be Solved?
This project was to create a required Reportable Event training that is easily consumed and meets the need for required training within 30 days of hire.

What will Success look like?
The Performance Goal was to ensure completion of the required training within 30 days of being hired by the Agency.

Who is the Audience?
The audience for this project are Agency non-medical caregivers with a high school (or equivalent) education, who work 20-40 hours per week in client homes with no dedicated field supervision.

What is the Solution?
In this case, a scrollable eLearning course which can be stored on the Agency's website, SharePoint or Google Drive was an easy, low cost solution.
